NSAC didn't get that, so they denied the request. They also wanted to know this type of information well in-advance before the fight. BEFORE THE FIGHT (and not just 90 mins before either, lol). I'll answer though.
1. Either they checked the box truthfully that night, or they were blatantly lying. They filled it out together and two heads are allegedly better than one. It's difficult to believe it was just a 'mistake.' The funny thing is Floyd, who allegedly can't read, didn't make the same mistake and even wrote down when the injury happened.
2. It's funny. Top Rank/Team Pacquiao are on record saying they told USADA. and USADA immediately and publicly denied knowing about the injury. And NSAC also said it's unacceptable to expect USADA to inform them.
3. I like the 'left it in the hotel room' part. That's a good one. It's funny they didn't have any verifying documents to show NSAC.
4. NSAC pretty much said the same thing. That Top Rank has been in the business for a long time. They know the process and how it works.
5. Either he was 100% or he wasn't. His physicians said he was fine and healthy and not a health risk, didn't need the meds. So for all things concerned, if he was not 100%, he was pretty close to 100%. And if he was not, who's fault was that? The funny part is that allegedly he had the injury for more than a month. That's half of training camp! What a joke!

Manny certainly had moments and certainly had the use of his right arm (he swung it repeatedly) but sadly it wasn't enough. His defense wasn't as sharp as it needed to be, he didn't cut off the ring like he needed to, and didn't throw enough combinations in the center of the ring. Manny threw combos mostly when Floyd was on the ropes but Floyd would parry, counter, block, spoil, or simply circle off the ropes.
Pacquiao fans just relax and take your loss like men instead of whining and complaining months after the fact like passive aggressive females
