He needs to get rid of that bag for starters. It is giving him bad habits like throwing with his hands low and not tucking his chin when he throws with the left.
I spar a guy regularly at the gym with the same height as this guy. He throws like him too. A good left hook or right uppercut is usually the slayer because he gets comfortably with his reach.
Tuck your chin and throw more from your lats not your hips

Ylem, you've improved a bit. Perhaps a heavier bag would suit you properly. I imagine it must be a pain-in-the ass when the bag keeps on moving wildly like that. Also, keep your hands up.

Plain and simple, forget about hooks and uppercuts.
Begin with the basics, jabs and right crosses only, or vise versa if you prefer south paw style.
Your bag isn't the main problem that people here keep stating, it's your form.
Just work on jabs and crosses.
You don't have to exert yourself either, do them slow and make sure your form is perfect with your jab and cross, THEN you can speed up and add power to the punch.
So that would consist of you doing about 40 or so jabs in a row and making sure each one is good form, then switch to throwing crosses with the same concept.
After that has been done, try going for a combo of a jab followed by a right cross with the same concept as the first.
