It's just the odds of it being a convenient excuse at a convenient time. There was many years when Pac didn't injure himself so bad, he had the option to decline the fight, but as the old way of the losers goes, make your excuses before you lose!
They always over-exaggerate something that happened in sparring, an accident etc when there is no physical sign at the time, but it's there, ready to bring back up when needed
Even though the medical examiners and physios have gave the greenlight and you tell them you are ok to fight

So what he's saying is Pacquaio voluntarily underwent nearly four hours of surgery for vanity. He lost the fight but c'mon with the "no injury" excuse there isn't a doctor in this country who would risk his/her license to help a boxer cover up an injury much less perform and unnecessary surgery.
