I don't recall whether or not there had been mention of a preexisting shoulder injury by Vitali prior to the Byrd fight, but full preparedness for battle should be expected and assumed in a sport where people can die. Following the fight he did have invasive surgery for a torn rotator cuff at Hamburg's Fleetklinik.
We will recall that younger brother Wladamir injured his left shoulder during his stoppage win over Ruslan Chagaev in June of 2009. Could anyone be mixed up? I do recall clearly that Vitali did have surgery on that shoulder after the Byrd fight. In the 3rd defense of the WBO title, he refused to answer the bell for the 10th round of the scheduled 12-rounder against Byrd, saying later he began to feel "fierce pain" in the third round.
This is, we might consider; a guy who has been front and center fighting the 2nd or 3rd strongest army in the world of late, and doing so while a personally identified assassination target of Vladimir *****.
He also underwent arthroscopic surgery in 2005 to repair a torn meniscus in his right knee. In 2012 he was diagnosed with a ligament tear in the left shoulder suffered during a 12 round decision win over Dereck Chisora. Manager Fritz Zdunek had stated then that the shoulder damage was a recurrence of the same injury Vitali suffered in the fight with Byrd in 2000.

I know Cunningham isn't Marciano, He weighed 210 against Fury's 254. This fight is to be seen as an example of how a bigger HW can wear down a smaller man.

At that size, if a man has the requisite talent and durability he is capable of beating anyone,imo. .
As you observed Cunningham isn't Marciano.
However,six feet three inches and two hundred and ten pounds, with an eighty three inch reach is rather different to five feet ten and a half and one eighty five/eight pounds,and a sixty eight inch reach.
There is a point at which size becomes a major factor
Cunningham was a good but not great fighter.
Probably good enough to beat most of Marciano's opponents,guys like ;
