Israel’s vaccine advisory panel has recommended a fourth dose of a Covid-19 shot for all adults, a world first, as the country battles a surge in infections driven by the Omicron coronavirus variant. The move follows research showing fourth doses doubled protection against symptomatic Covid-19 and increased protection against severe illness by three to five times, compared with three doses. Other countries, including the UK, US and Chile, have signed off on fourth doses for immunocompromised people, such as those living with HIV or certain transplant recipients, but have hesitated on launching a widescale second booster campaign. Israel’s vaccine advisory committee “recommends that those aged 18 and over be able to be vaccinated with a fourth vaccine dose”, confirmed the Israeli health ministry in a statement.

The scale of the number of children who have lost out on their schooling during the pandemic is “nearly insurmountable”, according to UN data.

Markedly increased risk in adolescents after 2nd dose may warrant refined vaccination strategies
A case-control study found that despite low absolute risk, there is an increased relative risk of carditis associated with BNT162b2 (commonly-known as Pfizer/BioNTech vaccine) vaccination. Considering the markedly increased risk in adolescents after the second dose, vaccination strategies may need to continuously consider the risk and benefits for different sub-populations, rather than taking a ‘one-size-fits-all’ approach. The findings will be published today (January 24, 2022) in Annals of Internal Medicine.

Unless they find a private clinic that is so unscrupulous as to proceed without vaccination because they are well paid and don't mind sentencing someone else to death that follows all the guidelines they are out of luck. Hopefully this guy will come to his senses and get vaccinated. His parents must be suffering over this.. Those conditions include being up to date on certain vaccinations. Committing to lifestyle changes and age. Considering how hard it is to obtain a heart for a transplant, doctors want to make sure as possible that the person recieving it has a chance for recovery. They do not want to waste a transplant on someone who has a poor chance of survival. After a heart transplant the person has almost a zero immune system. If they don't want to get vaccinated against Covid with Delta around and Omicron they are not a candidate to get a transplant. The same goes if they refuse to be vaccinated against hepatitis. On the flip side with zero immunity post operation there are certain vaccines that they cannot take or it most likely will kill them. The Rubella vaccine is one. The Covid vaccines are not restricted post operation here in Ontario. I'm guessing that is the same in other countries.

Either way the risk of myocardias from vaccination remains substantially lower than from getting them from COVID with the possible sole exception of the moderna vaccine in young males where it's modestly higher (about 40% off the top of my head) based on one study. And that's only one of many risks assocaited with COVID even ignoring potential future sequelae.

Nonetheless, although the estimated OR is large, the absolute risk for carditis is low, with the number of carditis cases per 100 000 doses of administered BNT162b2 estimated at 0.57. If we assume the carditis cases after CoronaVac vaccination is the background incidence—that is, 0.31 per 100 000 doses, the approximate absolute risk increase is 0.26 per 100 000 doses overall and 0.17 per 100 000 doses and 0.4 per 100 000 doses for the first and second doses, respectively. On the other hand, the risk for carditis and other serious complications after SARS-CoV-2 infection (), including an approximate 16-times increased risk for myocarditis shown in a retrospective cohort study of 36 million persons in the United States, are also concerning (). The risk seen in this study for carditis after BNT162b2 vaccination should be weighed against the benefits of vaccination.
Moreover, none of the 20 case patients with carditis after BNT162b2 vaccination were admitted to the ICU or died within the observation period, compared with 14 of 133 unvaccinated patients admitted to the ICU and 12 deaths. Our observations and published studies have shown that, to date, most reported carditis cases after BNT162b2 are self-limiting with good prognosis.
In conclusion, we observed an increased risk for carditis associated with the use of BNT162b2, particularly in young male persons receiving the second dose. Although the absolute risk is very low, this elevated risk should be made known to vaccine recipients and physicians and be weighed against the benefits of vaccination.

“There is a 1 in 3,000 risk for myocarditis (MIS-C) in patients with COVID-19,” , cardiologist director of quality in echocardiography and director of Kawasaki Disease Clinic at Children’s Hospital Colorado, and a professor of pediatrics at University of Colorado School of Medicine, told Healthline. “This risk is much higher than 1 in 50,000 to 1 in 100,000 in myocarditis associated with the vaccine.”
It’s that fact that has led scientists as well as regulatory bodies such as the Food and Drug Administration (FDA) to conclude that the benefits of receiving a COVID-19 vaccine of myocarditis.
“The cases of myocarditis following vaccination were mild with rapid symptom improvement,” said , vice chair of the Heart & Vascular Hospital at the Hackensack University Medical Center in New Jersey.
“The majority of the cases occurred in male patients between the ages of 12 to 39 years. The estimated rate of myocarditis following vaccination in males is approximately 41 cases per million persons. For females, the rate is 20 times less frequent,” he told Healthline.
Adults who develop than non-COVID-19 cases, including a higher risk of death. It should be noted that myocarditis associated with SARS-CoV-2 infection is just one of several heart conditions linked to COVID-19 with outcomes that are worse than non-COVID-19 cases.
In cases of myocarditis following COVID-19 vaccination, the vast majority of cases are mild and resolve quickly. In adults, . Similarly, in children, , and there has not been any reported need for mechanical heart support (extracorporeal membrane oxygenation, when blood is pumped outside the body to a heart-lung machine) or deaths. All children who had heart weakness had complete normalization of their heart function on follow-up.
Although this may not hold up completely for Omicron given it's generally lower level of serious illness. In fact the whole process of evaluating risk pretty much needs to start over and it'll likely be a few months before any proper Omicron studies start to filter through.

Taken from a recent study.
" They found that, during the first 12 months of the pandemic, males aged 12 to 17 were most likely to develop myocarditis within three months of catching covid-19, at a rate of about 450 cases per million infections.
This compares with 67 cases of myocarditis per million males of the same age following their second dose of a Pfizer/BioNTech or Moderna vaccine, according to figures from the US Advisory Committee on Immunization Practices.
The cases were mild in both groups but it's still a concern.

Ok so the estimated increased likelihood amounted to something like 1 in 250,000 patients and none of the p[ost vaccine myocardias recorded in Hong Kong required ICU treatment and none of them died. ie they were all relatively mild cases. By contrast in the unvaccinated control group there were 14 ICU admissions and 12 deaths.
FWIW they do conclude thusly which is a sentiment I can fully get behind... patients should definitely be given accurate information regarding the risks of any possible side effects prio to vaccination.
