Why not hit with the 2nd knuckles?

not the knuckles on top.
2nd knuckles are too weak.
i've swollen up my top knuckles punching.
using 2nd knuckles is improper punching.
you can break your fingers.

of all my years boxing, i've never gotten so much as a scratch, on my index knuckle

In Dempsey's book he gives an excellent exercise to demonstrate the power line.
- Stand in front of a wall arms length away with a closed fist
- Your fist should be placed on the wall at your own chin level (in terms of height and centering it) and just barely touching the wall.
- Now lean slightly forward and start placing some of your weight behind it so your fist begins to press more solidly against the wall.
What you will find is the first knuckle that makes contact with the wall is in fact your pinky knuckle on the outside of your fist which demonstrates where the power line runs and where the transfer of energy to your target should take place. This is true whether you do the exercise with your fist positioned so it is palm facing the floor or palm facing inwards.
Dempsey states that the pinky knuckle is the weakest along with being on the outside of your fist which makes it too fragile and vulnerable so you move one over and aim your punch with the knuckle on your ring finger and land with the last three knuckles (pinky, ring, middle) AKA Three Knuckle Punching
I always wondered about this while hitting the bag. I felt more comfortable hitting with the last 2, but the majority were hitting with the first 2 knuckles.
