Does anyone throw the pull-back counter right hand to the jab better than Mayweather?

Indeed. It's amazing that even the faded, heavyweight, C Cup having version of Toney was able to land that counter-right with regularity.

Hopkins looks at fighters' legs. He said he looked at Trinidad's legs throughout the fight to see when he was set.

Bernard Hopkins fights with his head down and guard around his ears. There are always exceptions to rules. Hopkins is far from a normal fighter tho. He's a master within his own right.

Ward is good at it too, but he lands elbows and butts instead

It's beautiful, so fluid. Floyd knows distance and timing.

cotto stopped it,because he jumps in with his jab

                    Yeah he definitely does.

I think opposing fighters know it is coming too. George Foreman (I believe) in one of Floyd's fights said about this stance: "it looks like he's a sprinter. looks like he's a sprinter on his mark." (something to this effect)

I think it's just incredibly hard to deal with Mayweather's reflexes and baiting with the constant head fakes. I realized that if a fighter doesn't commit to punching when Mayweather head fakes in this position that he will just start teeing off with lead rights.

The reason his pull counter is so effective is distance. Before he settles into this distance he gets off first almost every time forcing fighters to be aggressive to his feints and head fakes. That's why he's so frustrating, he beats you to the punch, then gives you different looks that seem more enticing and counters off these looks.
