I think the injury was real.
Nobody would fake that and risk earning good money for a year and a half. And kill their career at it at the same time.

Anyway he should just make his medical papers public to counter the doubters and to prove that his brain has healed sufficiently enough for promotors and broadcasters to take the risk of having him back.
