I've studied Ali in fights and interviews for decades and I must admit that I only saw the earliest symptoms (face masking and slightly slower speech on occasion) after the third Frazier fight. Not to say that fight definitely caused it, just that I never saw the slightest symptoms before it.

The problem is that the syndrome was inconsistent early on. There were times between 1975 and even the Holmes fight in 1980, where he could be as sharp and clear as before. But on the whole, the symptoms became more and more marked from 1976 (you can even catch the odd symptom in his movie 'The Greatest'. Circa 1976).

I seemed to notice it slightly in a Howard Cosell interview I was watching leading up to the Chuck Wepner fight following the Foreman fight. So it seemed to start a little then and get noticeably worse a few fights later after the third Frazier fight.

        - -Not just speech, it was the marked loss of mobility and defense.

People closest wanted him to retire , but by then he'd become an industry of team and guaranteed title fights whereas in his early days he had problem making fights because no venue wanted to deal with the NOI problem.
