I watched Eyes without a Face a few months back...weird stuff loki. It wasn't as grotesque as I expected.

Anti christ and Necromantik as well

I've seen Necromantik and I nearly died from laughing when the guy was stabbing himself/jerking off/cumming blood. I'm not a fan of any movie like C.H. that shows animal abuse or real death. I don't know why people would want to see that.

I see someone already mentioned Anti-Christ 2009, it ****** but what a sick movie. But if you want to see a real twisted film watch Martyrs(subtitled) and Inside(subtitled). Both are French horror films, you might want to watch it alone, you don't want anyone walking in on you watching it

any film by Jose Mojica Marins Brazilian director from 70's--80's did alot of wild stuff most if not all was terribly bad but sorta entertaining

"Jenifer" comes to mind. Takes a lot to disturb me, but that movie did. I have read good things about Takashi Miike's imprint, been meaning to see it. Miike has some pretty intense stuff, Ichi the Killer is worth checking out.

Eyes Without a Face»Les yeux sans visage (original title)

A brilliant surgeon, Dr. Génessier, helped by his assistant Louise, kidnaps nice young women. He removes their faces and tries to graft them onto the head on his beloved daughter Christiane, whose face has been entirely spoiled in a car crash. All the experiments fail, and the victims die, but Génessier keeps trying

Good movie, watched that a few years back. Billy Idol based a song on that movie, the video is hilariously great.

I have to mention Martyrs as well! This movie had me feeling numb for a few days.

If you want total over the top, anything goes, gross out horror comedy then you want to watch the Feast trilogy. These movies go above and beyond and are totally unpredictable as far as who lives and who dies. Here is a sample of what you are in for from Feast 2 and I warn you, it is pretty disturbing...
