The mouthpiece is still protecting his bottom teeth most people (including Floyd) have a slight over bite so when they where a mouthpiece the top row of teeth is protected and it overlaps/cushions the gap between the top and bottom. For those whose teeth are perfectly aligned,meaning top and bottom rows front teeth meet evenly, a "double mouthpiece" is usually what is preferred...
(That's not to say a double mouthguard can't be used regardless of under/over bite but it's just more practical in that instance )
In other words even if he knocked his bottom tooth out it's highly unlikely, as long as his mouth remained closed, that it would've made it past his mouthpiece. Let alone fly out the ring...

If it opens up, like how his mouth opened when Maidana threw the punch. The teeth in the bottom with no mouth guard becomes unprotected. Hence the tooth flew off and evidence is in the picture. A missing tooth on one side.
Ain't pulling anyone's leg here. Proof is there for you to see..video tells the story. Sorry for telling the truth I know it's painful to see your idol getting hurt..
