This is definitely a freak accident:
Except for the time it happened with Angulo vs. Lara.
Oh, and the time with Shane Mosley vs. Fernando Vargas, and Jesus Soto Karass vs.Andre Berto, and Guillermo Jones vs. Denis Lebedev, and Erik Morales vs. Marcos Maidana, and Manny Pacquiao vs. Antonio Margarito, and Omar Figueroa vs. Nihito Arakawa, and Delvin Rodriguez vs. Pawel Wolak, and Mauricio Herrera vs. Ruslan Provodnikov, and Nigel Benn vs. Chris Eubank.
I could go on and on, but I think my points are pretty clear :
1.) Severe swelling happens all the time in boxing.
and
2.) You're a moron.

Actually it was the other way around: given the mat was soft, small ring size, hydration limits in the contracts, everything was set up to so Angulo would look good against a reputed boxer. But then Lara never got the script.

Vanes refused the rematch because he complained that he would need one extra week for his cut to heal. I don't remember how far off the projected rematch was from that fight but basically Vanes didn't want to rematch due to one week, instead opting for a fight on a Macau untelevised undercard.
