I have a hard time swallowing the drug theory, but I have to admit that something definitely looked off. However, that could also be explained by simple panic-induced fatigue resulting from the realization that he was about to fail again.
All that aside, Vlad's confidence was sundered in the Sanders fight, and it hasn't resurfaced yet. It could, in this upcoming fight.
I think this fight will play out one of two ways. In one possibility, Castillo comes in hard, puts Vlad on the hot-spot yet again, and wins the fight by a TKO against Vlad who's yet again humiliated in the ring.
The other possibility is that Manual Stewart has been working with Vlad on his defense, and helping him to regain his confidence, and that VLad has come to the understanding that fear of failure has no place in a boxing ring, and comes to the ring focused, and puts an extremely early stop to Castillo.
Personally, I think the second option is what will play out on the 23rd. Vlad will come back looking like someone poured 2 quarts of fresh guts down his throat, and closes the show in an exciting fashion before the ending bell of round 2.
